Work Package 6
Management strategy
Building on needs identified in WP1 and the technology standards developed in WP2, this work combines social, medical, and digital approaches to better translate cardiovascular disease (CVD) evidence into real-world care.
The aim is to improve prevention and management for people whose treatment does not currently meet guidelines, helping reduce long-term CVD deaths. Across participating cities, WP6 will develop solutions suitable for wider use across Europe and evaluate their effectiveness and implementation challenges.
Leaders
Work Package 6 is co-led by the University of Birmingham and Servier with support from Novartis.
Objectives
1
Identify scalable existing interventions that can deliver better CVD management for underserved populations.
3
Deploy interventions for primary and secondary prevention of adverse cardiovascular outcomes, with robust evaluation of relevant metrics to quantify the step-change achieved.
2
Iterate and refine potential interventions using health technology to support consistent implementation of guideline-directed CVD management.
4
Design strategies for clinical guideline adherence, transition of care and implementation across different cities.
